We formed a partnership with the Beach Haven Moose Riders 4 years ago at our Inaugural Frank V. Panzone Jr. Memorial Cruisin for a Cure motorcycle and classic car event. We had horrible weather that day with blowing winds, cold temperatures and sideways rain. However, the Beach Haven Moose Riders made the decision to ride no matter the conditions because they understood our patients affected by a cancer diagnosis needed their help and they were not going to let them down.

Social Security Disability Benefits for Cancer

Have you been diagnosed with cancer? One of the last things on your mind should be making ends meet. Fortunately, there could be financial resources available for you and your family. The Social Security Administration (SSA) offers disability benefits for people in need.

The primary form of disability benefits is known as Social Security Disability Insurance, or SSDI. In order to be eligible for SSDI, you have had to work throughout most of your adult life and paid a sufficient amount of taxes to SSA. So long as you’ve worked part-time for any five of the past ten years, you will almost certainly qualify for SSDI benefits.

While many cancers have a good prognosis thanks to advanced technology and medical advances, your treatment may keep you from working.

Medical Qualifications and the Blue Book

The SSA has strict guidelines when it comes to determining an individual as disabled. There is a medical guide, which is known as the Blue Book, they use to determine whether or not you are considered medically as disabled and meets the qualification for disability benefits. There are some cancers that meet Blue Book approval based on advancement of the cancer, while others are approved based on the diagnosis alone.

Some cancers may not initially qualify with just a diagnosis, but may be approved if the cancer has “metastasized,” or spread. For example, breast cancer that has spread to the collarbone, colon cancer that has spread to the pancreas or liver, or lung cancer that has spread to the kidneys would qualify for disability benefits. The location and severity of the cancer as well as its response to treatment and your symptoms and side effects are all taken into consideration. If your cancer has returned despite a round of treatment, you will almost always medically qualify for benefits.

Cancers That Qualify by Diagnosis Alone

There are some cancers that automatically qualify for Social Security disability just by diagnosis. If you are diagnosed with any of the following cancers, you will automatically qualify for SSDI benefits:

  • Esophageal
  • Pancreatic
  • Spinal Cord Carcinoma
  • Mesothelioma of the Pleura
  • Inflammatory Breast Cancer
  • Small Cell Cancer of the Lungs
  • Liver Canccer
  • Primary Cancer of the Bile Ducts
  • Primary Cancer of the Gall Bladder
  • Thyroid Cancer

 

Qualifying for Disability Using a RFC

If you do not meet the Blue Book listing for your form of cancer, you could still be approved for disability benefits. You will need to have your doctor fill out a residual functioning capacity (RFC) on your behalf. An RFC form defines your limitations clearly, so disability determination services can understand what you are able to do and what you are no longer able to do. Your physician will complete a RFC and state your limitations and how your symptoms are impacting your ability to work and handle your daily activities. You can download an RFC online.

For many kinds of cancer, the SSA will need to consider your disability using a medical-vocational allowance in which a disability examiner or a disability judge decides whether a claimant is able to participate in work activity at a sufficient amount to earn substantial and gainful income. This is a process in which any past work activities and experience in addition to any other kinds of work that the SSA believes you may be able to do are considered based on your limitations and functional capacities.

In addition to the limitations brought on by your cancer and your cancer treatment, any long-term physical side effects brought on by the treatments, such as cognitive and memory disorders and slowed thought processes will also be given consideration. Your age, education level, and work skills are also considered when determining whether or not you are disabled and eligible for benefits.

Applying for Social Security Disability Benefits

You need to supply the SSA with all of your medical records, physician notes, documentation of your symptoms and how they affect you, test results, and any surgical notes. You do not need to physically give the SSA these forms, as the SSA will gather your hospital records on your behalf. All you will need to do is list all the doctors you’ve worked with and hospitals where you’ve been treated.

Before applying for disability benefits, it is wise to speak with your oncologist. Your oncologist will be able to offer advice as to how you can qualify for benefits. He or she can fill out additional paperwork or statements on how your upcoming cancer treatments prevent you from working to assist your claim. When you have doctors working with you, your chances of approval can be greatly improved.

In the event that your cancer spreads or progresses, you should notify the SSA as soon as possible. If you are interested in applying for Social Security disability benefits, you can call the SSA toll-free at 1-800-772-1213 or visit the SSA’s website at www.SSA.gov.

We held our 4th Annual Island Gala “Hope Under the Stars” on Friday August 5th at the Sea Shell Resort & Beach Club. It was a beautiful night filled with HOPE Under the Stars!

The most critical information about David’s Dream & Believe Cancer Foundation (DDBCF) was conveyed to the crowd by Co-Founder, David Caldarella during the opening ceremony. 634 Families battling cancer have been supported by DDBCF over the past 5.5 years. 312 families in the first 4 years of existence and 322 in the past 1.5 years. A group of friends set out to assist 1-2 patients affected by a cancer diagnosis per year. DDBCF is currently on pace to offer solely financial assistance to a patient/family every 1.46 days in 2016 and if you add in the emotional support and hope – that number rapidly drops to 1 patient/family per day! Truly an incredible milestone for the DDBCF Family and the supporters that make this “Generosity of Spirit” possible throughout the year! THANK YOU!

The highlight of the evening was hearing from Co-Founder of David’s Dream & Believe Cancer Foundation (DDBCF); Tim Hall, Co-Chair of the Department of Otolaryngology at Thomas Jefferson University Hospital; Dr. David Cognetti and 3 of the new Directors Patient of Advocacy for David’s Dream & Believe Cancer Foundation: Dawn Aversa, Susie Buzby and Tina Angers. Tim spoke about the origin of DDBCF and in particular a story of Hope when his friend of 40 years David Caldarella was at his darkest point during his battle with stage IV head and neck cancer and one of his best friends Carmen Malatino created a CD of recorded voicemail’s from family, friends and Dr. David Cognetti encouraging David to Dream & Believe. Dr. Cognetti talked about his personal relationship with his patient David Caldarella and his recollection of the early stages of DDBCF all the way through today. Dawn, Susie and Tina spoke about their personal experiences with cancer as a caregiver in Dawn’s case and survivor’s in Susie and Tina’s and the positive impact DDBCF has had in their lives.

We are honored to officially announce: The Inaugural Frank A. Cosimano Memorial Golf Classic will take place at White Beeches Golf & Country Club on Tuesday October 11, 2016. The Cosimano Family and Co-Founder David Caldarella and his family both grew up in north jersey but spent every summer and most off-season weekends together on Long Beach Island. The Cosimano’s are family to David and Mr. Cosimano was like a second dad. As many of you know, Frank A. Cosimano died on August 7th 2011 from Pancreatic cancer. Frank loved his wife Angela, family, friends, animals, golf and most importantly helping others. He was passionate about giving back to individuals and charities. One charity that was very close to Frank and remains close to his family is David’s Dream & Believe Cancer Foundation. We will honor and remember this great man as we join together for The Inaugural Frank A. Cosimano Memorial Golf Classic in this the 5th year of his passing to serve as “A Beacon of Hope” for all those families currently battling a cancer diagnosis.

Finding A Calling In His Darkest Hours: David’s Dream & Believe Cancer Foundation

David Caldarella is a stage IV head and neck cancer survivor. He’s also a sort of angel-on-earth for many who know him. And a hero. A leader and visionary. A friend. A listening ear. An inspiration. Perhaps most significant to the hundreds of people he has helped, he’s the Co-Founder of David’s Dream & Believe Cancer Foundation (DDBCF), which serves families affected by a cancer diagnosis.

HIS BATTLE
The first sign of David’s illness occurred in March, 2010. After dining out with a friend, David reports, “We returned home with me having some difficulty swallowing. It felt as if something were stuck in the back of my throat. When I went to shave in the next day or two, I noticed a lump on the right side of my neck.” David’s first doctor’s appointment in early April was rapidly followed by another, and then a surgery to remove a tumorous growth on his right tonsil and to biopsy the lump on his neck.
When his doctor confirmed that David had Squamous Cell Carcinoma, “The words seemed to hold in the air for a while, just long enough for me to watch my mom’s eyes well up with tears.” David was immediately referred to Thomas Jefferson University Hospital (TJUH) in Philadelphia, and he was scheduled for a PET scan.
The PET scan showed that the cancer spread from David’s tonsils, down his throat, and into the lymph nodes on the right side of his neck. The time leading up to that major test was also a turning point for David. “I realized in those moments prior to the scan that there was a real possibility I was going to die. I made peace in that moment with that possibility. But I also made a conscious decision: I would fight like hell. And I wanted to create a nonprofit so that the loves of my life—my nephew, Timothy, and two nieces, Isabella and Sabrina—could be proud of their uncle. Somehow we could help other families affected by a cancer diagnosis.”
But first there would be treatment. The plan consisted of surgeries, radiation, and chemotherapy. David writes, “The surgeries were rough on me. I was hospitalized a few times for bronchitis and kidney stones. I never had the opportunity to heal and was forced to start radiation and chemo thirty days after my last surgery.” This was the treatment plan’s recommended timeframe to ensure the best possible results.
David goes on, “Radiation and chemo proved to be difficult foes. My overall health began to plummet. I went from 190 lbs. at the start to 125 lbs. at the end of treatments. I suffered second degree burns on my neck from radiation. My mouth was a war zone from the combination of surgeries, radiation, and chemotherapy. It was unbearable, even with every major form of pain medicine. Relief was a fleeting concept.”

HIS DREAM
David’s intense and difficult treatment and its complications were sometimes alleviated by envisioning a future for his nonprofit. He writes, “The pain was severe and constant during my treatment, but so was the support from family, friends, and even strangers. My strong faith in God helped me through some of the darkest of times… The concept of creating a nonprofit to pay it forward proved to be my strongest ally in my battle with stage IV cancer. There were far too many sleepless nights and restless pain-filled days over the next few months, but it was in those same times that creating this Foundation helped me find some solace despite my discomfort.” David considered major, formative questions: “How would we describe our mission? What would we call it? Who would volunteer to serve on the Board of Directors?”
My dream was that I was going to defeat stage IV cancer, but I had to believe in that dream.”
The DDBCF received its official 501(c)(3) tax-exempt status a few days after David’s last radiation treatment. Its mission and goal: to help one or two families affected by a cancer diagnosis annually. As for what to call it, that answer came naturally. David explains, “A friend of mine suggested using the website Caringbridge to disseminate information about my current health status. It was an easy way for me to keep everyone updated without having to respond to calls, texts, and emails. I always signed my updates with, ‘Dream & Believe, David.’ The name for the Foundation originated from this, David’s customary sign-off.
David was informed he was cancer-free on November 11, 2010. He remembers thinking, “This is the best day of my life.” Within a few weeks, DDBCF started fundraising with a pizza party at the Ship Bottom Fire House in December 2010, earning almost $10,000. David’s leadership and vision, along with the strength of its Board of Directors, enabled the Foundation to begin and continue its important work. He writes, “We were able to offer financial assistance to 20 families in 2011 and 50 in 2012. In 2013 and 2014, we were able to assist 117 and 120 respectively. In 2015, we offered financial assistance, services/programs and – most importantly – HOPE, to 212 families, in the amount of $155,181.66.”
Although these successes far exceeded the Foundation’s initial expectations, the team understands deeply why and how there is always more to do on behalf of families battling cancer. David writes about some of those the Foundation helps: “There’s the husband/father concerned about putting food on the table for his family, so he stops taking his expensive chemo pills; there’s the couple whose two-year-old daughter is diagnosed with cancer and starts losing the ability to sustain their business because they’re constantly driving back and forth to hospitals; there’s the single mom who is trying to care for three children when the youngest is diagnosed with cancer; there’s the senior citizen couple or senior individual with nobody, left to fend for themselves during the darkest of times; there’s the happily married couple with two kids and one on the way, only to find out the pregnant mom has developed cancer.”

HIS TEAM
Making DDBCF a reality and growing it to unexpected success started with bringing David back to health. David continually mentions by name, and with much admiration and gratitude, the health practitioners who have and continue to support him. David clearly made significant connections with his treatment team despite the haze of illness and discomfort that hung over him during that time.
Others stepped up to push forward his dream of a nonprofit, serving due to their relationship with David and their passion for the Foundation’s mission. Tim Hall is Co-Founder of DDBCF and is currently serving as Treasurer after almost five years as President. David writes, “Tim has been like a brother of mine for almost 40 years. He made it a point to travel to New Jersey six different times during my cancer battle to spend time with me and take me to treatments.” Since Tim lives in Kentucky with his family, this was no small task. David says that at DDBCF, Tim “constantly reminds us that our mission must stay focused on our patients and their families. Our patients make it very easy for us to do that; they inspire us on a daily basis with their faith, courage, and determination.”
Kelly Powers served on the Board of Directors in various roles including Secretary and Treasurer for almost four years. David has known Kelly for 25 years. For more than half of his 33-day radiation treatment plan, Kelly drove from her home in Trenton to get David in Manahawkin and take him to TJUH in Philadelphia, then took him back to Manahawkin before returning to Trenton. Kelly’s friendship and continued support for the Foundation remain a constant source of strength for David.
Kim McCabe Manzella currently serves as President on the Board of Directors and is responsible for creating and managing the youth group, Generation Dream. Kristin Panzone is on her second two-year term and serves as an Event and Artistic Director. Dani Corso is in her first term and is currently serving as Secretary and Communications Director. Steven Crisalli joined the Board of Directors in January 2016. All of the Directors serve on a 100% volunteer basis.
David’s girlfriend, Catherine Deely, is the Foundation’s top advocate, and can be found volunteering her time to support David and the mission year-round. Joan and Vincent Caldarella, David’s parents, are very proud of David’s caring spirit: “We are humbled to witness the work of David’s Dream. His mission truly is one of mercy and love for all the people that have walked in his tracks.” David’s sister Christine, brother-in-law, Dan, nephew, Timothy, and nieces, Isabella and Sabrina, have always provided David with support, shoulders to lean on, and smiling faces.

HIS FUTURE
Buoyed by the gift of cancer-free status and supported by the commitment and generosity of the Foundation’s leadership, supporters, and volunteers, David continues to pursue the mission. He has spoken to over 500 families and individuals dealing with a cancer diagnosis, and notes that while each may experience a different and unique battle, as humans, they share hope and a will to live.
For DDBCF, supporting patients’ and family members’ hope and will is foremost; they strive to do so through fundraising and community outreach. One newer initiative includes the Foundation’s youth group, Generation Dream. This effort gives kids between the ages of six and 16 opportunities to raise awareness for DDBCF, create fundraising projects, and mostly, David suggests, to “generate a ripple effect of kindness for New Jersey families battling a cancer diagnosis.” The Foundation recently created a Patient Advocate Committee consisting of their patients, caregivers and Nurse Navigator Elizabeth Martinez from Southern Ocean Medical Center. Lizzie was also responsible for nominating David for the inaugural Hero of Hope Award from the Academy of Oncology Nurse & Patient Navigators which David did win in October 2015. David and Kim serve on the Southern Ocean Medical Survivorship Committee and David currently serves as a Patient Advocate on the Head & Neck Steering Committee and Patient Advocate Steering Committee for the National Cancer Institute. DDBCF has also created a student group at Southern Regional High School — where David’s nephew Timothy is a member — and the Foundation has enlisted the support of Professor Dr. Ai Zhang and her students from Stockton University.
David mentions something that summarizes his and the Foundation’s story perfectly: “I read something during my battle with cancer: Kindness is the shadow of God in man. Those beautiful shadows were never more present than during my battle with cancer. That same kindness, generosity of spirit, faith, hope, and love are the core beliefs of DDBCF.” David’s story shows us that both light and shadows can precede the creation of something important.

600 Families & Over $450,000.00 in Grant Donations! 

Our first event was a party with food donated by Panzone’s Pizza at the Ship Bottom Volunteer Fire Company in December 2010. We assisted our first patient named Molly (13 years old) who was diagnosed with Osteosarcoma in early 2011. Our initial goal was to provide financial assistance to 1-2 families per year who were affected by a cancer diagnosis. Our original theme was, “Saving Lives by Fighting Cancer Together.” We were able to assist 20 families in 2011 with another 50 families supported in 2012. The increased demand for support from families battling a cancer diagnosis in our local community and the New Jersey community at large skyrocketed in 2013. Our mission statement and focus was steadfast from day one – to offer financial assistance to our families. As we pursued this mission we began to see the “little engine that could” called David’s Dream & Believe Cancer Foundation transform into a vehicle of HOPE for our patients and their families. Our renewed theme became, “A Beacon of Hope.”

The past three and a half years have resulted in an additional 530 families being offered financial assistance, services and hope! A total of 600 families & over $450,000.00 in grant donations are staggering numbers on so many levels. However, it’s our personal connection to each of our patients and their families that inspire us on a daily basis.

This monumental giving would not be possible without the support of so many and have to start with recognizing the original Board of Directors including Co-Founder Tim Hall who remains an active member of the Board and is currently serving as our Treasurer. Our Board of Directors past and present serve on a 100% volunteer basis. They all have their own families, work and other charitable commitments yet still choose to dedicate time to serve our families. Our business sponsors and individual donors have made it possible for us to offer financial assistance to 600 families and have invested themselves in our mission to help countless more. Informed Mortgage in Manahawkin is the shining example of this giving spirit as they created a planned giving initiative with guidance from our Director Dani Corso to donate $25.00 from every closed loan out of their Manahawkin office to DDBCF and our families battling cancer. This creative planned giving initiative resulted in a 2015 year-end donation of $18,000.00 to DDBCF. We have the most amazing volunteers who commit their time and energy throughout the year to support an event or activity that benefits our families battling cancer. We have our own youth group called Generation Dream created by Kim McCabe Manzella as well as student groups at Southern Regional High School and Stockton University. The 7 members of our brand new Directors of Patient Advocacy represent the 600 patients and families we have supported over the past 5+ years. All 7 members have been affected by a cancer diagnosis and have been directly impacted by David’s Dream & Believe Cancer Foundation. They all have chosen to serve as advocates and Beacons of Hope!

I read something during my battle with stage iv head and neck cancer: “Kindness is the shadow of God in man.” Those beautiful shadows were never more present than during my battle with stage iv head and neck cancer in 2010 but they have reappeared time and time again through the mission of David’s Dream & Believe Cancer Foundation.

The 5th Annual Stafford Run for Hope presented by Informed Mortgage to benefit David’s Dream & Believe Cancer Foundation (DDBCF) was the first year DDBCF was involved in the entire event. It was a very good first year and look forward to many more successful Run for Hope events. The original Stafford Triathlon/Duthlon was created by our friend and Race Director Ross Reynolds. Ross and his wife Marissa put forth a yeoman effort and we cannot thank them enough for supporting DDBCF. A special thanks to Chuck and Danielle Schnell and all the volunteers!

Our presenting sponsor Informed Mortgage in Manahawkin, NJ continue to set the bar very high. Informed Mortgage’s planned giving initiative to donate $25.00 per every closed loan out of their Manahawkin office was started last year. The planned giving continues today with the end result being a tremendous financial support system for our families battling cancer. Thank you Informed Mortgage.

We had 2 Main Sponsors for this year’s Stafford Run for Hope event. The Law Office of William Wright LLC in Manahawkin and Thomas Jefferson University Hospital in Philadelphia. William Wright competed in the this year’s events and took home some award hardware. We want to thank our friends at Thomas Jefferson University Hospital. ER Tech Mike Rowe is also the Founder of 11th Hour Racing and led the triathlon relay team to a first place finish in the relay division. The team consisted of Dr. Rich Massone in the swim, Dr. Katie O’Leary on the bike and Mike on the run. Mike formed this nonprofit to assist patients at TJUH in need of financial assistance. We look forward to partnering with Mike and the entire TJUH in the future!

We want to thank our student group from Southern Regional High School led by teachers Rebecca Posch and Jody Denn McLennan. We had more than 20 SRHS students volunteering on the day of event and additional students participating in the race. Great job & Thank You!

We are beyond excited for the 4th Annual Island Gala to benefit: David’s Dream & Believe Cancer Foundation at the Sea Shell Resort & Beach Club on Friday August 5th, 2016. This year’s theme is “Hope Under The Stars!” Our patients and families affected by a cancer diagnosis inspire us on a daily basis. This year’s Gala will be very special as we introduce some of our brand new Directors of Patient Advocacy. This is a group of individuals directly affected by a cancer diagnosis and positively impacted by the support from David’s Dream & Believe Cancer Foundation.

The beautiful Sea Shell Resort & Beach Club is once again the venue for this year’s Gala. The Hughes Family and staff treat this event as if it were a special event for a family member. We appreciate their continued support and friendship.

Our entertainment lineup for the 4th Annual Island Gala is second to none! We met American Idol Alum Cara Samantha a few years ago as a result of her cousin Jamie being diagnosed with cancer. Unfortunately, Jamie eventually lost her courageous battle with cancer. Cara will be performing during cocktail hour and under the stars in our outside “Hope Tiki Garden” too. A brief opening ceremony will take place after cocktail hour. Then everybody should get ready to dance the night away to Philadelphia’s High-Energy Seven Piece Band: Love 77. The members of LOVE 77 are motivated by the mission of the Foundation and ready to bring the house down with some incredible Live Music! Our favorite Emcee, Live Auctioneer and Friend: James Donahower is back to host the entire evening for us this year.
